- 4Use a standard GP communication template at the end of every review to confirm changes and follow-up plan.
- 5Document patient counselling — purpose of changes, side effects to watch for, and when to seek help.
- 6Run focused pharmacist/clinician feedback on high-risk medicines and local monitoring requirements.
- 7Review cases with high anticholinergic burden and agree local deprescribing priorities with pharmacy and frailty teams.
- 8Set a re-audit date after implementation, using the same denominator and criteria for direct comparison.
