NICE NG5: Medicines optimisation; NHS structured medication review guidance; STOPP/START criteria version 3
Eligible adults receiving multiple regular medicines should have a structured medicines review that documents medicine burden, high-risk medicines, anticholinergic burden, renal function, falls risk, medicine indication, patient/carer involvement, STOPP/START or equivalent appropriateness criteria, actions and follow-up.

Standing principles
- This tool supports, but does not replace, clinical judgement.
- Follow local polypharmacy, medicines optimisation and deprescribing policy; seek pharmacist, geriatrician or specialist input where needed.
- High-risk medicines, renal impairment, falls risk and high anticholinergic burden should trigger appropriate senior/pharmacy review and monitoring.
- Audit identifiers should be pseudonymised — do not record patient-identifiable data.
